Sorry you're having difficulty applying the Aldara! Sounds frustrating.
One thing about Aldara is it doesn't have to be applied "perfectly" on the area where warts are, but I don't know exactly how close to the warts one must get. Aldara seems to work by provoking a response from your immune system, at the "cellular level", in the area where the cream is applied.
You're right in that it's not indicated for internal warts, and your doctor must think the warts to be removed are in a location where you can reasonably expect to apply the cream effectively. Ask him/her if you have questions about the location of the warts and the proximity of where you apply the cream, and maybe a pharmacist can help, as well.
