I have been in a long distance relationship for about two years. We have been together for weekends maybe ten times. I have had HSV-2 for 20 years, no ob for years, and am on suppression therapy. No condoms. I am going to have the talk next time I see her. Poor judgment, I know.

What are the chances of transmission?

The average rate of transmission from male to female ( assuming this is the situation - if it isn't, please let me know ) with suppressive therapy and avoiding sex during obvious genital symptoms is about 4-5%/year. It's a relatively low risk overall.

But that assumes a couple living together, right? If the frequency of relations is lower, than the risk should be lower too it seems. Maybe it is more like 1% in this case? On the other hand, I guess what's the difference because it is more than 0%.

It's based on an average of having sex twice a week, not on living together. The valtrex and reduction in transmission study is where the info comes from - NEJM Jan 1, 2004.
